Ownership

Pure IV is a physician-owned company. The following individuals are the owners of Pure IV:

Name Title Role
Allison Lane, MD Medical Director Physician owned Company

Pure IV's physician-owned structure means that the medical protocols, treatment formulas, safety standards, and clinical policies are developed and maintained by the physicians who own and operate the company. Medical oversight is not outsourced to a third-party compliance platform or contracted medical director service. The physicians who own Pure IV have a direct and personal stake in every patient outcome and every clinical decision made under the Pure IV name.

Medical Direction and Clinical Oversight

All medical services provided by Pure IV are delivered under the medical direction of:

Allison Lane, MD - Medical Director

Allison Lane, MD

Medical Director

License Number: NPI #1417269648

Dr. Lane is responsible for the oversight of all clinical services, treatment protocols, and patient safety standards across all Pure IV operating markets. Her responsibilities include developing and approving all IV therapy treatment protocols and formulas, establishing clinical guidelines for patient screening and treatment, overseeing the Nurse Practitioners who conduct live patient assessments before every treatment, supervising the clinical performance of all bedside providers, approving the credentialing and onboarding of all clinical staff, reviewing and updating medical protocols as clinical evidence and regulatory requirements evolve, and ensuring compliance with applicable state and federal healthcare regulations in every market where Pure IV operates.

Dr. Lane's medical direction is direct and ongoing. She is not a contracted medical director assigned through a third-party platform. She is the physician who oversees the medicine practiced under the Pure IV name and who is personally accountable for the clinical standards maintained across the organization.

Nurse Practitioner Assessment

Before every treatment, a licensed Nurse Practitioner (NP) conducts a live, real-time medical assessment of the patient via phone televisit. This assessment is a Good Faith Examination as required by applicable state law and serves as the basis for establishing a patient-provider relationship and approving a patient-specific treatment plan.

The NP reviews the patient's health intake, medical history, current medications, known allergies, and present symptoms. The NP evaluates whether the requested treatment is safe and appropriate for the patient's current health status and, if so, issues a patient-specific clinical order. If the NP determines that the treatment should be modified, an alternative should be recommended, or the treatment should not be administered, the NP communicates this decision to the patient and the bedside provider during the live assessment.

The Good Faith Examination is conducted at every appointment, not only at the first visit. There is no separate charge for this assessment. It is included in every Pure IV appointment at no additional cost to the patient.

All Nurse Practitioners providing assessments for Pure IV are licensed in the state(s) where their patients are located, operate within their scope of practice as defined by the applicable state board of nursing, and practice under the medical direction of Dr. Allison Lane, MD, in accordance with applicable supervision and collaborative practice requirements.

Bedside Provider Credentials

All IV therapy treatments are administered by licensed healthcare providers. Pure IV employs only the following categories of licensed professionals to administer IV therapy:

Provider Type Licensure Scope
Registered Nurse (RN) Licensed by the state board of nursing in the state where services are provided. Holds active, unrestricted RN license. IV placement, administration, and monitoring. Vital signs assessment. Patient education and informed consent. Adverse event recognition and response.
Paramedic Licensed or certified by the state EMS authority in the state where services are provided. Holds active, unrestricted paramedic credential. IV placement, administration, and monitoring. Vital signs assessment. Patient education and informed consent. Adverse event recognition and emergency response.

Pure IV does not use IV technicians, medical assistants, emergency medical technicians (EMTs), or unlicensed personnel to administer IV therapy. All providers hold active state licensure or certification and maintain current professional liability (malpractice) insurance.

Every provider undergoes a credentialing process before treating patients under the Pure IV name. This process includes verification of active state licensure or certification, verification of professional liability insurance, background screening, clinical competency assessment including IV placement proficiency, and training on Pure IV's clinical protocols, documentation standards, and emergency procedures.

Your bedside provider remains with you for the duration of your entire infusion. Providers do not leave patients unattended during treatment.

The specific contents of each treatment package are listed on our website at pureiv.com/iv-packages-pricing. Your Nurse Practitioner may modify the formula for your specific treatment based on your health profile, medications, allergies, or clinical assessment during your live televisit.

Telehealth Disclosure

Pure IV utilizes telehealth (live phone televisit) technology to conduct Nurse Practitioner patient assessments before every treatment. The following information is provided in compliance with applicable state telehealth disclosure requirements.

The telehealth assessment is conducted via a live, real-time phone call between the patient and a licensed Nurse Practitioner. The NP is licensed in the state where the patient is located at the time of the assessment. The patient's bedside provider is present during or immediately following the televisit. The telehealth platform used by Pure IV operates under a Business Associate Agreement (BAA) and complies with HIPAA privacy and security requirements.

You have the right to refuse the telehealth assessment and request an in-person evaluation before treatment. If you decline the telehealth assessment, Pure IV may not be able to provide treatment at that appointment, as the NP assessment is a required component of every appointment.

You have the right to discontinue the telehealth assessment at any time. You have the right to ask questions about the telehealth process before it begins. The same standard of care applies to telehealth assessments as applies to in-person assessments. Your telehealth interaction is subject to the same privacy protections as an in-person medical encounter, as described in our Notice of Privacy Practices at pureiv.com/privacy-practices.

Informed Consent

Before any treatment is administered, Pure IV obtains informed consent from every patient (or from the parent or legal guardian of a minor patient). The informed consent process includes a review of the treatment plan approved by the Nurse Practitioner, an explanation of the contents of the IV or IM injection, a discussion of the potential risks, side effects, and benefits of the treatment, an opportunity for the patient to ask questions and receive answers, and the patient's voluntary agreement to proceed with treatment.

You have the right to refuse or withdraw consent for any treatment at any time, including during treatment. If you wish to discontinue an IV infusion that has already begun, inform your provider immediately and they will safely discontinue the treatment.

If treatment is refused, discontinued, or declined by the NP or provider for medical reasons, you will not be charged for any service that was not provided.

Risks and Limitations of IV Therapy

IV therapy is generally considered safe when administered by licensed medical professionals under appropriate medical supervision. However, as with any medical procedure involving the insertion of a needle into a vein and the introduction of substances into the bloodstream, there are inherent risks.

Potential risks and side effects include but are not limited to pain, discomfort, or bruising at the IV insertion site, minor bleeding, inflammation or irritation of the vein (phlebitis), allergic reaction to one or more components of the IV solution (including, in rare cases, anaphylaxis), infiltration (IV fluid leaking into surrounding tissue), infection at the IV site, lightheadedness or dizziness, nausea or metallic taste, changes in blood pressure or heart rate, fluid overload (particularly in patients with heart or kidney conditions), and in rare cases nerve damage, air embolism, or serious injury.

No specific outcome, result, or therapeutic benefit is guaranteed from any Pure IV treatment. Individual results vary based on the patient's health, hydration status, nutritional status, and other individual factors.

Pure IV's safety protocols, including the live NP assessment, vital signs check, licensed providers, and continuous monitoring, are designed to minimize risk. However, no medical procedure is entirely without risk.
