How Diphenhydramine Works

- Nausea support. Diphenhydramine has antiemetic properties. It's sometimes used alongside other anti-nausea medications for patients experiencing significant nausea from illness, hangovers, or migraines.
- Migraine protocols. Emergency departments commonly use IV diphenhydramine as part of migraine ‘cocktail’ protocols. It helps with associated nausea and has mild sedative properties that can help relax patients during an active migraine.
- Premedication. Diphenhydramine may be given before certain IV medications to prevent potential allergic reactions (similar to how hospitals premedicate before some treatments).
- Insomnia and anxiety during illness. Diphenhydramine’s sedative properties can help patients who are unable to rest due to illness-related discomfort.

Side Effects and Safety
Diphenhydramine is well-studied and generally well-tolerated. The primary side effect is sedation — which is sometimes the intended effect and sometimes an accepted tradeoff for symptom relief.
Common side effects:
- Drowsiness/sedation. This is the most common effect and the reason Benadryl is also marketed as a sleep aid (ZzzQuil). Plan not to drive or operate machinery after receiving IV diphenhydramine.
- Dry mouth. From the anticholinergic effects. Your IV fluids help counteract this.
- Mild dizziness. Usually resolves within an hour.
Who should avoid IV diphenhydramine:
Patients with narrow-angle glaucoma, urinary retention, or severe prostate enlargement should inform their provider. Elderly patients may be more sensitive to anticholinergic effects. Our NP screens for all contraindications before administration.
